      摘 要:目的:观察髂后上棘微调矫正法治疗骶髂关节后下错位的临床疗效。方法:将2020年5月-2021年4月于黑龙江中医药大学附属第三医院就诊的90例骶髂关节后下错位患者随机分为观察组、对照A组和对照B组,每组各30例。三组在关节整复前均采用传统推拿手法进行基础治疗以放松肌肉,随后观察组采用髂后上棘微调矫正法,对照A组采用传统复位手法,对照B组采用美式整脊矫正法。所有基础治疗均1次/天,手法矫正为1次/2天,7天为1个疗程,共治疗2个疗程。分别于治疗前、治疗后及治疗后1个月随访时观察三组患者VAS疼痛评分、ODI功能障碍评分、腰椎JOA评分和骨盆两侧髂嵴高度差的变化,比较三组的临床疗效差异。结果:治疗14天后,观察组的总有效率为93.3%,明显高于对照A组的70%和对照B组的60%。与治疗前相比,三组患者治疗后和治疗后1个月随访的VAS评分、ODI评分和两侧髂嵴高度差均明显下降,JOA评分明显升高(P<0.05)。在治疗后和治疗后1个月随访时,观察组VAS评分、ODI评分和两侧髂嵴高度差均低于对照A组和对照B组,且JOA评分高于对照A组和对照B组。结论:对于骶髂关节后下错位患者,髂后上棘微调矫正法能能明显缓解患者骶髂关节疼痛,改善机体日常功能,提高腰椎活动程度,其临床疗效和远期疗效均明显优于传统复位手法和美式整脊法,值得临床推广应用。

      Abstract: Objective: To observe the clinical effect of fine-tuning correction method of posterior superior iliac spine in the treatment of posterior-inferior dislocation of sacroiliac joint. Methods: From May 2020 to April 2021, 90 patients with posterior lower sacroiliac joint dislocation who were treated at the Third Affiliated Hospital of Heilongjiang University of Chinese Medicine from May 2020 to April 2021 were randomly divided into observation group, control group A and control group B, each with 30 example. The three groups used traditional massage techniques for basic treatment to relax muscles before joint restoration, and then the observation group used the posterior superior iliac spine fine-tuning correction method, the control group A used the traditional reduction method, and the control group B used the American chiropractic correction method. All basic treatments were performed once a day, manual correction was 1 time/2 days, 7 days was a course of treatment, and a total of 2 courses were treated. The changes of VAS pain score, ODI dysfunction score, lumbar JOA score and iliac crest height difference on both sides of the pelvis were observed in the three groups before treatment, after treatment and 1 month follow-up after treatment, and the differences in clinical efficacy of the three groups were compared. Results: After 14 days of treatment, the total effective rate of the observation group was 93.3%, which was significantly higher than 70% of the control group A and 60% of the control group B. Compared with before treatment, the VAS score, ODI score and bilateral iliac crest height difference of the three groups of patients after treatment and 1 month after treatment were significantly reduced, and the JOA score was significantly increased (P<0.05). After treatment and 1 month follow-up after treatment, the observation group's VAS score, ODI score and bilateral iliac crest height difference were lower than those of control group A and control group B, and the JOA score was higher than control group A and control group B. Conclusion: For patients with sacroiliac joint posterior and lower dislocation, the posterior superior iliac spine fine-tuning correction method could significantly relieve the patient's sacroiliac joint pain, improve the daily function of the body, and increase the degree of lumbar spine activity. Its clinical and long-term effects were significantly better than traditional ones. The reduction technique and American chiropractic were worthy of clinical application.
